DEPARTMENT:

The Pharmacy services operate over four sites within Alfred Health and provide high quality, innovative, patient-focused services, education and training-based programs as well as high quality research.  Pharmacists are essential members of all clinical teams and have a critical role in patient management. The service has three key priorities:

  • Medication safety
  • Effective use of medicines
  • Delivery of specialist pharmacy expertise at critical points in the patient care pathway

POSITION:

The Lead Pharmacist – Home, Acute and Community Services is responsible for the oversight and leadership of the delivery and development of clinical pharmacy services across the Rehabilitation, Aged and Community Care Services of the Home, Acute and Community clinical program of Alfred Health. This includes the supervision and development of a number of Senior Clinical Pharmacists and their teams. The services encompass approximately 100 inpatient beds and a range of community programs.

Based principally at Caulfield Hospital, the services include acute and integrated care, Health of Older People Programs and Rehabilitation services. Home, Acute & Community (HAC) is a clinical program across Alfred Health. The program plays an integral role within Victoria’s health care system, supporting people to maximise their health, independence and functioning, and minimise long-term care needs.  The Home, Acute and Community program oversees a diverse group of services promoting effective seamless care, from acute care back into the community. The services target people with loss of function or ability from any cause, either congenital or acquired. The Home Acute and Community Program is a state-wide provider of rehabilitation for patients following acquired brain injury, spinal cord injury, multi-trauma and amputation.

The Lead Pharmacist will also provide direct clinical services to the Aged Care unit, and have dedicated non-clinical time to support quality development of the team. Research is an essential part of pharmacy services at Alfred Health. The post holder would also be expected to contribute to the research and educational activities of the HAC Program as well as pursue their own related program of research.

 

QUALIFICATIONS AND EXPERIENCE:

  • Registered pharmacist with AHPRA
  • Relevant clinical post-graduate qualification such as Masters in Clinical Pharmacy Practice or equivalent
  • Completion of a relevant specialist qualification, or completion or progression towards a PhD highly desirable
  • Recognition as an ANZCAP consultant, or equivalent, in the specialty of Transitions of Care, Geriatric medicine and or Leadership and Management highly desirable

 

DESIRABLE SKILLS:

  • Extensive experience in the provision of clinical pharmacy services in rehabilitation, aged care, geriatrics, community care services or a related specialty.
  • Extensive hospital pharmacy experience with a demonstrated understanding of contemporary practice in hospital clinical pharmacy services, QuM and therapeutics.
  • Extensive experience in development, delivery and evaluation of clinical pharmacy services.
  • Extensive experience in supervision, mentoring and development of staff.
  • Experience in development of senior staff highly desirable.
  • Superior administrative and organisational skills, and exhibit the ability to work and communicate effectively with pharmacy staff and other health professionals.
