The Department

The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department. The Social Work Service includes approximately 52 EFT (operational reports) with an additional 70 EFT (professional reports) who provide interventions to inpatients and outpatients across all Alfred Health clinical programs. The Social Work Service aligns to the Alfred Health strategic goals and operational performance targets. Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services that increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.

The Position Summary

The Social Worker will be responsible for the provision of advanced social work services to patients and their families/significant others. This role will require an experienced Social worker with sound skills to support both Alfred Health’s; General Medicine acute inpatient unit and Hospital in the Home (HITH) program. This role allows for social work to work uniquely in supporting patients across the continuum of care by supporting patient’s transition home after an admission to hospital. General Medicine at Alfred Health comprises of a team of doctors, nurses and allied health professionals who provide acute care for a wide variety of conditions for patients within the metropolitan area and across the state. The ability to conduct home visits will be required for this role. 

Skills and Qualifications Required

  • Eligibility for membership of Australian Association of Social Workers. Bachelor of Social Work, or recognised equivalent
  • Required to perform work that requires special knowledge and depth of experience in any one or more of the following:
    - Family Violence/ Child Protection/ NDIS/VCAT Guardianship List 
  • Comprehensive knowledge and counselling skills related to chronic illness, illness behaviour and adjustment to illness, loss and grief
  • Knowledge of current Legislation and Policy in relation to Social Work in a hospital setting
  • A current Victorian drivers license is required for this role
