About us
The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department. Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to the community.
About the role
We are seeking a Social worker to work within various areas across Bayside Health - Alfred Care Group. The successful candidate will need to have hospital experience and be able to work within a strong multidisciplinary team. The candidate will need to have a good understanding of working within an Electronic Medical Record. The successful candidate will need a good understanding of social work theories and apply them to hospital practice. It will be essential to have a sound understanding of complex discharge planning within an acute hospital setting in particular an understanding of Transition Care Program, My Aged Care and the NDIS. An understanding of home based care is crucial for this role. The successful candidate will need excellent communication skills to collaborate with patients, families, other disciplines and community stakeholders. This is a fast-paced environment and requires timely and quality discharge planning and the successful candidate will need to work flexibly, creatively and collaboratively.
About you
- Applicants must hold a Bachelor Degree in Social Work, or recognised equivalent.
- Applicants must have an understanding of the psychosocial issues that patients may be affected by, within a hospital setting.
- Applicants will need to have had experience in working within a hospital setting

- It is vital that the successful candidate has experience in working within an Electronic medical record.
