Full-time 80 hours a fortnight, includes ADO's
Monday-Friday, no weekend work.
HARP Complex Care is a community-based service within the range of Health Independence Programs (HIP), and in Alfred Health sits within the Community Integration Program.
This Grade-Two Social Work position works within a site based geographical team alongside nurses and complex care coordinators. The social work case manager is responsible for undertaking comprehensive bio-psychosocial patient assessments and developing and implementing patient-centered care plans in collaboration with the patient, family, carers and broader multidisciplinary team.
The position works in a dynamic context in a multidisciplinary model to provide an integrated flexible model of care. HARP works in collaboration with a multitude of stakeholders and the patient to improve the health and wellbeing of individuals through clinical and psychosocial interventions
The successful applicant will have:
Grade Two Social Work capabilities.
Sound assessment skills, the ability to recognise risk and or deterioration in the community and escalate appropriately
A strong interest in caring for people who experience significant challenges in both chronic disease, vulnerability and psychosocial complexities
A Bachelors Degree in Social Work
