The Department

The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on the quality of life and participation in care is a primary goal of the Department.
Under the direction of the Manager of the Social Work Department, the following services are provided:

  • Social Work

Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services that increase the quality of life and participation in care. Social Work also assists patients and their families transition smoothly from hospital to home.

The Role

This is an exciting opportunity for an experienced Social worker with advanced social work skills and relevant hospital experience to provide leadership and clinical coverage in General Medicine.  The role will assist in leading a small team and in providing a client-centred approach to assisting patients and families who require counselling, advocacy and support throughout the patient journey.   The role will include conducting comprehensive psychosocial assessments, liaising with the interdisciplinary team and working with families and external stakeholders to coordinate and facilitate safe and effective discharge plans.  Advanced Social work skills are essential in this role for managing complex, challenging or sensitive issues.   The candidate will need to have experience in providing supervision and will need to have experience supervising staff and students. The candidate will need to have a sound understanding of the patient cohort in General medicine.  A sound understanding of data collection and the ability to manage team priorities and caseloads and be autonomous at times whilst ensuring critical KPIs are met will be essential for this role.  This position aims to reduce the length of stay, prevent admissions to inpatient wards, coordinate hospital and community services, prevent re-admissions to hospitals and improve patient outcomes and consumer satisfaction. This role will require the successful applicant to have relevant team leadership experience and will require the applicant to have experience leading a small team.  Current experience with student and staff supervision is crucial for this role.  This role will also require the successful candidate to work one weekend day per month.  

Skills/Qualifications Required 

  • Demonstrated experience in conducting comprehensive psychosocial assessments
  • Highly developed liaison and advocacy skills
  • Highly skilled in conflict resolution

Essential

  • Previous experience working within a hospital setting
  • An understanding of working with an Electronic medical record
  • Knowledge of NDIS, Aged Care services will be advantageous  
  • The ability to work a weekend day will be required for this role
