Department

Home, Acute and Community is a clinical program across Alfred Health. The program plays an integral role within Victoria’s health care system, supporting people to maximise their health, independence and functioning, and minimise long-term care needs. The Home, Acute and Community program oversees a diverse group of services promoting effective seamless care, from acute care back into the community. These services are provided in people’s homes, community centres and in hospital when indicated. The Mobile Assessment and Treatment Service (MATS) is part of the Health of Older People stream, and is funded as a Health Independence Program [HIP]. HIP services aim to increase health independence for clients, their carers, and significant others. Position Summary

The Outreach Nurse works at the interface between acute and sub-acute hospital, GPs and RACF’s, to meet the needs of our vulnerable patient group. They are the “face of MATS” in the community, and are expected to represent the values of Alfred health & MATS service with a professional and courteous attitude. Work is underpinned by the aims as listed above, and always in line with the patients or their medical treatment decision makers expressed wishes. Their practice is overseen by MATS Team Leads and NUM, with tasks delegated by the team care coordinators and medical staff as required. Clinical oversight is also provided by the Geriatricians working within MATS. The MATS Grade 3B Outreach Nurse is an integral part of the MATS service, and requires a dynamic nurse to optimise this role.

Qualifications and Experience Required

  • Current Division 1 professional registration with the Nursing and Midwifery Board of Australia.
  • Four years or more of clinical experience in Emergency, Critical care or General medicine
  • Sound clinical knowledge of complex health and medical conditions
  • Demonstrated ability to work positively and flexibly with internal and external services.
  • A high standard of written and verbal communication skills
  • Demonstrated interpersonal skills
  • Commitment to continuing professional development
