Alfred Health is a leader in health care delivery, improvement, research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, from ambulatory to inpatient and home and community-based services. On 1 January 2026, Alfred Health will come together with Kooweerup Regional Health Service, Peninsula Health, Bass Coast Health and Gippsland Southern Health Service to create Bayside Health, a newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians. The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.

Department
Alfred Mental & Addiction Health (AMAH) is responsible for the operation of services which focus on people with a mental illness residing in the Inner South Eastern area of Melbourne.
The newly integrated Adult and Older Adult Service combines AMAH Adult Community Mental Health and Caulfield Aged and Liaison Psychiatry Services to provide comprehensive, person-centred support for adults and older adults. We offer a wide range of therapeutic, addiction, trauma-informed, and family violence services.
In line with the Royal Commission into Victoria’s Mental Health recommendations and the Victorian Mental Health and Wellbeing Act 2022, we are expanding our services to provide specialized expertise and broad-spectrum care, including pathways for substance use, rehabilitation, recovery, and trauma-focused care.
Our approach focuses on integrated treatment, education, peer support, care planning, and navigation, supporting individuals at every stage of their recovery journey.

About the role -
Community Pathways Support (service name TBC) is a new AMAH service providing transitional clinical care for consumers stepping down from tertiary services or needing support to connect with primary healthcare providers. Delivered by a multidisciplinary team at St Kilda Road Clinic, the service operates across two care streams (GP Shared Care and Transitional Care), with flexibility for consumers to move between them should their needs change and rapid re-entry to tertiary services if required.
The Senior Clinician will work alongside the Community Transition Liaison Coordinator to triage and allocate referrals, manage a caseload across both streams, and provide clinical leadership when needed, education, quality improvement, and service development while collaborating closely with internal and external stakeholders, carers, and families.
About you
- Registration as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
- Bachelor of Nursing with completed Mental Health Post Graduate Diploma
- Significant full-time (5+ years) senior clinical experience in delivering mental health-based interventions including case management and demonstrated leadership
- Master of Nursing or health related Masters is desirable
