About us

Alfred Health Renal Service provides comprehensive renal care across inpatient, outpatient, and community settings, including dialysis and transplantation. Services span acute and chronic kidney disease and are delivered through multiple sites, including Caulfield, Sandringham, and partner locations like Peninsula Health. Dialysis options include in-centre, home, and satellite units, and transplant services cover deceased, live donor, and multi-organ transplants.

About the role

The Renal Transplant Nurse Practitioner (NP) works autonomously and collaboratively as part of a multidisciplinary renal transplant team—including nephrologists, surgical and nursing staff, and allied health professionals—to deliver comprehensive, appropriate, and timely care to patients across the entire renal transplant continuum. This includes pre-transplant assessment and preparation, post-transplant follow-up, and long-term management.

Focusing on patients with advanced chronic kidney disease (CKD) undergoing evaluation for transplantation, as well as post-transplant, the NP plays a pivotal role in optimising outcomes. Responsibilities include clinical assessment, diagnosis, prescribing treatment, managing immunosuppressive therapy, and providing regular patient reviews. Through expert clinical knowledge and an extended scope of practice, the NP addresses service gaps by enabling more frequent, convenient, and locally accessible care.

About you

  • Current Registration as a Registered Nurse with Nursing and Midwifery Board of Australia via AHPRA.
  • Minimum of 5 years experience in Renal nursing
  • Post graduate Masters of Nurse Practitioner qualification
  • Motivated, autonomous with an ability to work collaboratively with departments across many health services and community
  • Proven clinical leadership skills and a commitment to quality and safety and research activities
