About the Team

Alfred Mental & Addiction Health (AMAH) is responsible for the operation of services, which focus on people with a severe mental illness residing in the Inner South Eastern area of Melbourne.

The single most important goal of AMAH is to create an environment that facilitates clinical recovery, supports individual recovery efforts and strives to minimise service system barriers to the recovery process.

The Continuing Care Team (CCT) as part of the community mental health team utilises a case management model to provide centre based and outreach assessment, treatment, and support services to clients generally aged 25 -65 years who experience a serious and/or enduring mental illness, and who require specialist clinical services. The CCT staff aim to work in partnership with consumers and carers, enabling them to be involved in their care in order to achieve the best possible outcomes. The staff of the CCT are clinically divided into five multi-disciplinary sub-teams, each with its own Consultant Psychiatrist, discipline representation, lived and living experinece workers and allocated client case load.

About the Role

As our Practice Lead - CMHT, you will work in collaboration to the Manager- CMHT to support the team to deliver high quality, evidenced based interventions, care and support services offered to our consumers, including day to day support, upskilling and problem solvng, and coordination for the team.  You'll also be a key member of the senior leadership team, providing support for the flow of referrals, promoting staff welfareand the monitoring of incident reports as the occur.

Skills & Experience

  • Approved tertiary qualifications and registration with the Occupational Therapy Board of Australia via AHPRA, or eligibility with the Australian Association of Social Workers (AASW)
  • Australian working rights
  • Full Covid-19 and 2024 Influenza vaccinations
  • Current Victorian Driver's License and be willing to undergo a Background Check
