Grade 4 Complex Care Social work Lead
Permanent Full Time- SC53- 80 Hours + ADO

The Department
The Social Work Department operates in accordance with the philosophy and goals of Alfred Health. A focus on the quality of life and participation in care is a primary goal of the Department.
The Role
The Grade 4 Complex Care Lead (CCL) Social Worker will be a recognised clinical expert by peers across the service both internally and externally to the profession and will motivate and inspire others. They will foster innovation and excellence in the delivery of Social Work services to Social Work staff across all Alfred Health campuses. The CCL Social Worker will provide high quality, evidence-based Social Work services and will be responsible for leading complex clinical care for patients across the lifespan with multiple medical and psychosocial comorbidities.
This is an exciting new role that involves leading the Social work team across Alfred Health. The role will involve looking at the ever changing needs of the service in identifying gaps in complex care management and evaluating and supporting the delivery of complex Social work support across Alfred Health. The successful applicant will have proven leadership experience and be able to understand the complexities involved in delivering patient centred care in a high risk setting. The CCL will also need to have extensive knowledge in working in a hospital setting and have an advanced understanding of working in the Acute and Sub Acute health setting. The CCL will need advanced skills in grief and loss counselling and bereavement support. Skills and experience in identifying and intervening early with issues of risk are required (for example, Family violence, Child safety and the NDIS). The CCL will also support senior clinicians to manage high level patient complexities within the health service. In this role, the CCL will be required to develop capabilities in the Senior Social work workforce and will support the senior workforce with career development and progression. Bedside coaching and support for Social workers will be a key requirement of this role and experience in bedside coaching is essential. This role will sit within the Social work department at Alfred Health and will provide support to Social workers at all Alfred Health campuses (The Alfred, Caulfield & Sandringham Hospitals).

Qualifications & Experiences Required
- Approved degree in Social Work and evidence of eligibility for membership of the Australian Association of Social Workers (AASW).
- Advanced social work skills and experience as evidenced by proven, demonstrated post-qualifying experience and a demonstrated commitment to ongoing professional development.
- Knowledge of current hospital social work practices.
- Evidence of involvement in and/or understanding of principles of research and evaluation.
- Current Victorian Drivers License.
- A current Working with Children’s check is required to undertake this role
- Post Graduate qualifications at a minimum of a Masters level or working towards completion including a demonstrated interest in continual learning opportunities and development.
- Proficiency in working with an electronic medical records will be essential to this role.
