The Department
The Mobile Assessment and Treatment Service [MATS] is funded as a Health Independence Program [HIP]. HIP services aim to increase health independence for clients, their carers, and significant others. The Mobile Assessment & Treatment Service (MATS) is overseen by Geriatricians, and encompasses MATS Medical and Nursing roles, with access to Allied Health services. The service provides rapid clinical in-reach to residents in residential aged care facilities [RACFs] and the community who are at risk of presenting to emergency departments for reasons that could be managed outside of the hospital setting. The MATS patient cohorts are the frail and elderly, or those with severe disability in residential settings. Often MATS patients have a range of chronic conditions and conditions of ageing.
The Role
The MATS Team Lead is an experienced operations leader that plays a pivotal role in supporting the day to operations of the service to anticipate and respond to issues arising. This position works closely with the MATS Manager to optimise clinical outcomes for residents in residential aged care facilities.
- This role works at the interface of acute/sub-acute hospital, GPs, and residential facilities.
- The Team Lead is a senior team member with oversight of MATS G4A & 3B RNs, and accountability for coordinating the day to day clinical activities in the MATS service.
- The role involves liaison between health care professionals both in the community and Alfred Health, and within the organisation.
- This role is responsible for monitoring, anticipating and responding to issues arising in the day to day operations of the service.
- This role acts a clinical resource and leads change and systems improvements
The position requires excellent communication and documentation skills to foster effective relationships with internal
- and external stakeholders to the patient’s care.
Qualifications and Experience Required
- Current professional registration with the Nursing and Midwifery Board of Australia as a Registered Nurse (Division 1)
- Post graduate qualifications are highly regarded
- Health leadership experience
- Excellent written and verbal communication skills
- Commitment to continuing professional development.
- Current Victorian Driver’s Licence
