The Integrated Care Team (ICT) is an outward-facing General medicine Community Team within the range of Health Independence Programs (HIP).  ICT sits within the Alfred Home - Acute and Community (HAC)  and reports through the Community Integrated Stream. 



This position is open to an experienced registered nurse that has a strong interest in chronic and complex holistic health care. This role works within The General Medicine ICT team, working alongside the ICT resident and Consultant. The General Medicine Liaison Nurse  is responsible for providing direct nursing care, client education, and participating in care coordination for clients being treated by the Integrated Care Team (ICT) within General Medicine. As a Liaison Nurse, you understand the importance of building relationships not only between yourself and the client, but also the direct medical team, primary care providers and the wider multidisciplinary team, in order to obtain the best outcome for the client. 

The General Medicine Liaison Nurse will facilitate and interdisciplinary care approach in the following 3 settings:

  1. Home based care- Home visits to provide direct nursing care or in collaboration with ICT medical team. This may also take the form of Telehealth reviews and incorporate remote patient monitoring.
  2. Centre based care- Assessment of health care needs and nursing interventions provided in an outpatient clinic setting
  3. Hospital based care – Liaison with ward based general medical teams to provided expert advice and support for complex care patients referred into the Integrated Care Team on discharge.


Qualifications and Experience


  • Bachelor of Nursing.
  • Current professional registration with AHPRA
  • Four years or more nursing experience.
  • Clinical experience in Emergency Medicine, Critical Care and/or General Medicine.
  • Aptitude for delivering client education using a motivational approach.
  • Demonstrated ability to work flexibly with internal and external services.
  • A high standard of written and verbal communication skills
  • Demonstrated interpersonal skills
  • Commitment to continuing professional development.
  • Current Victorian Driver’s Licence
