Alfred Health Carer Services (Carer Services) support the people who care.

Carer Services is funded by State and Commonwealth Governments to deliver programs including phone support, in person support services, peer support, information and social activities to carers (including young carers) of people who are aged, living with dementia, have a disability or chronic illness, and/or mental illness.  We provide services to support carers and care relationships in a client-centred, timely and responsive manner.  Carer Services is part of Alfred Health’s Home, Acute and Community program.  



The Team Leader: CAP: DMHYC is responsible for developing and maintaining high quality services to carers. This includes leading and developing staff expertise and delivering services to ensure high practice standards, fulfilment of funding targets and obligations, maintaining awareness of sector developments, developing strategic partnerships, and managing service demand.

The DMHYC team, incorporates the CAP team and Carer Wellbeing team.  The CAP team delivers assessment, planning, monitoring and service linkage for carers (including young carers) of people under 65 years who have a disability, chronic or mental illness. The Carer Wellbeing team provides extended support, carer coaching, in-person peer support and carer assessment and planning functions as required.  Initially, the Team Leader will only be responsible for the CAP team as we transition to a new service model.

QUALIFICATIONS/EXPERIENCE REQUIRED (see position description for full list)

  • Tertiary qualifications in Health, Behavioural Science, Social Sciences or Welfare
  • Minimum 3 years’ management experience in a community or health setting
  • Experience in health, government or services sectors
  • Experience managing staff, including the ability to advise and support staff in service provision and develop team performance
  • Energy, enthusiasm and commitment to create and promote high quality support services to clients
  • Capacity to work independently and autonomously, yet to facilitate a collaborative culture
  • Demonstrated understanding and awareness of issues facing carers and people from diverse communities
  • Successful applicants must undergo a police check and have a current Working with Children Check
  • Current Victorian Driver Licence
