DEPARTMENT Alfred Mental and Addiction Health is responsible for the operation of services which focus on people with a severe mental illness residing in the Inner South Eastern area of Melbourne.
The objective of this position is to build an effective operational interface between Alfred Mental and Addiction Heath and the National Disability Insurance Scheme (NDIS), to ensure consumers and carers are supported to access and participate in the NDIS. This will include –
Establishing the mental health Program Lead position as the key contact point for the National Disability Insurance Agency (NDIA) local office and the NDIS Local Area Coordinator (LAC) in the service catchment.
Building an effective partnership between Alfred Mental and Addiction Health and the NDIA local office, LAC and NDIS funded service providers.
Building the capability of Alfred Mental and Addiction Health’s workforce (including clinicians and lived experience staff) to proactively engage and support consumers and carers throughout the NDIS access, planning, plan implementation and review processes.
Facilitating a coordinated response to shared consumers, particularly in response to a rapid escalation in the person’s psychosocial support needs and coordinated discharge planning from bed-based acute services.
Liaising with DHHS regarding NDIA related reporting and initiatives.
Relevant AHPRA registration OR experience in community capacity building and advocacy related to disability and the NDIA
Demonstrated experience working in the human services sector
Demonstrated understanding of mental health policy and legislation
Understanding of the NDIA and NDIS principles
Demonstrated clinical experience in particular in relation to assessment and planning
Strong knowledge of the principles of person-centred care and ability to apply
