Caulfield Hospital, Health of the Older Person Program
Permanent /Part- time role - .08 EFT (32 hours per week)
The Alfred Health’s Caulfield Hospital is seeking the services of an innovative, energetic and motivated social worker to join our interdisciplinary Health of the Older Person team and Social Work Department.

The Social work department at Caulfield Hospital provides support to patients and families. Caulfield Hospital specialises in community services, rehabilitation, aged care and aged mental health. The hospital plays a state-wide role in rehabilitation services, which includes the Acquired Brain Injury Rehabilitation Centre, neurological rehabilitation, spinal rehabilitation and care for amputee patients.
In conjunction with the Leadership Team the position is responsible for the day to day operational functioning of the designated Social Work team across the Aged Care and Rehabilitation wards. Currently all these wards are amalgamated and are co-located in one area. These comprise of ACG, AC1, AC2 and AC3. The Senior Clinician has a key role in providing clinical leadership and specialist advice to Social Workers and staff to assist in the management of complex cases and discharge planning. This role will require you to manage a caseload on the ACG dementia ward as well as providing high level support to staff to manage any complex cases that may escalate within the service.
- Ability to lead a team including managing workload allocations and supervision of staff
- A high level understanding and knowledge of working with an Electronic medical record
- High level conflict resolution skills
- A sound understanding of Aged care and Rehabilitation processes including NDIS, VCAT and TCP processes
- A high level understanding of managing day to day operational processes
Applicants must hold a Bachelor Degree or Qualifying Masters in Social Work
Previous experience working with aged care patients and working within a hospital setting.
Current relevant leadership experience in managing a team within a hospital setting is essential for this role
