The Department of Infectious Diseases at the Alfred Hospital is one of the largest and most comprehensive infectious diseases clinical services in the country. It is also an academic Department within the Faculty of Medicine, Nursing and Health Sciences, Central Clinical School, Monash University and is a premier centre for clinical and biomedical research and education. The Victorian HIV Service, which is the leading HIV service in the state, sitting within the Department of Infectious Diseases it is a leader in HIV clinical care, research and education.
This role will be a key nursing role within the Victorian HIV Service. They will lead the expansion and development of the HIV Nursing Model of Care within the Infectious Diseases Ambulatory Service. The model will be developed in partnership with the HIV Leadership Group, Infectious Diseases Physicians and the Infectious Diseases Ambulatory Nurses.
The HIV Nursing Model of Care will involve working in partnership with Infectious Diseases Physicians in the Outpatient Clinic in an integrated model. This will involve clinical audit of individual patient’s history and HIV database utilising a tool, undertake nursing assessments and developing an Outpatient Plan for each patient in partnership with the Specialist leading the patients care.
The aim will be to support the service to meet best practice guidelines in clinical care across all HIV patients attending the outpatient clinic. In addition, the role will be a key contact point for the doctor and patient to facilitate referrals and coordinate other aspects of the patients care requirements post the Outpatient visit. The role will utilise advanced HIV nursing skills to lead aspects of the patients care, including assessment, audit, proactively pre-empt clinical needs and determine appropriate interventions and responses.
The role will work closely with the HIV multidisciplinary team. They will lead the development of nursing clinical care processes and oversee care pathways relating to HIV care such as management of patients lost to follow up.
QUALIFICATIONS AND EXPERIENCE:
- This role will be well suited to an experienced HIV nurse or an experienced ambulatory nurse working outside of HIV
- Current registration with the AHPRA
- Post graduate qualification in HIV/Infectious Diseases or other relevant field is desirable but not essential Experience managing a Clinical Service
- At least 7 years clinical practice and strong professional identity
- Experience in a senior HIV clinical nursing role
