DEPARTMENT

This position sits within the Alfred Health Home Care Packages Team based at Caulfiled Community Health Service.

ROLE

The position works as part of a motivated team responsible for the provision of high quality case management services to Home Care Package clients.    The Allied Health Assistant works closely with Case Managers and contracted Service Providers to coordinate a flexible and responsive service that endeavours to meet identified client/carer/family needs with in- home support workers, and within specified budgets

RESPONSIBILITIES 

  • The role supports Case Managers with day to day client-related tasks and includes accountability for client care co-ordination in achieving individual client and team goals. 
  • The Allied Health Assistant works closely with Case Managers and contracted Service Providers to coordinate a flexible and responsive service that endeavours to meet identified client/carer/family needs.
  • The role supports Case Managers with day to day client-related tasks and includes accountability for client care co-ordination in achieving individual client and team goals. 
  • The role maintains electronic client records in an accurate and timely manner.
  • Comply with legal, organisational and professional standards of practice
  • Achieve individual and team Key Performance Indicators and targets in order to meet funding requirements
  • Actively contribute to Caulfield Community Health and PAV Response Service quality improvement activities and initiatives.

KEY CAPABILITIES: ESSENTIAL

  • Certificate IV in Allied Health Assistant or equivalent relevant qualification
  • Excellent communication skills (verbal and written), and organisational skills
  • Excellent Information, Communication and Technology  (computer) skills
  • Demonstrated ability to provide high quality person centred care
  • Demonstrated capacity to understand and adhere to organisational policies, guidelines and work instructions
  • Well-developed collaboration, problem solving and time management skills
  • Excellent presentation and professional manner
  • An ability to work independently
  • Current Victorian Driver’s licence                                                                        

DESIRABLE     

  • Experience in working within the health and / or community sector
  • Experience in working in a multi-disciplinary team
  • Awareness of the Home Care Packages program, policy and guideline requirements.
