DEPARTMENT

Hospital Admission Risk Program (HARP) sits under the Community and Ambulatory Service. HARP cares for people with chronic disease, aged and/or complex needs who frequently attend hospitals or are at risk of hospitalisation.  HARP’s Respiratory Service provides assessment, monitoring and treatment of clients with Chronic Lung Disease.

ROLE

This position would suit a physiotherapist who is looking to expand or apply their knowledge of respiratory care in the community for complex clients through Pulmonary Rehabilitation (PR) and HARP.

PR is run from two sites, The Alfred and Caulfield, as well as providing a home-based rehab program. This role would primarily work at Caulfield Hospital and in the client's homes. The position will involve group exercise and education sessions, and individual sessions with respiratory patients who are participating in the program.  Effective communication skills and the ability to work in a multidisciplinary team are essential.

SKILL

To be successful you will:

  • Be responsible for the provision of high quality physiotherapy service across all clinical areas within the HARP
  • Have a good theoretical knowledge of management of respiratory disease and the Pulmonary Rehabilitation program
  • Very good clinical reasoning/ decision making skills in the clinical areas
  • Excellent verbal communication and negotiation skills to support effective liaison with clients, their carers and other health professionalsb
