  • Full time, 80 hours/fortnight + ADO

The Department

The Social Work Department operates in accordance with the philosophy and goals of The Alfred.  A focus on quality of life and participation in care is a primary goal of the Department. Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.

 

The Role 

We are seeking a Social Worker to join our Social Work team in General Medicine at The Alfred. You will have relevant hospital experience and be able to work within a strong multidisciplinary team. To be successful, you will need to have a good understanding of working within an Electronic Medical Record and, exhibit an understanding of social work theories and apply them to hospital practice. It will be essential to have a sound understanding of complex discharge planning within an acute hospital setting in particular an understanding of Transition Care Program, My Aged Care and the NDIS. The successful candidate will need excellent communication skills to collaborate with patients, families, other disciplines, and community stakeholders. This is a fast-paced environment and requires timely and quality discharge planning and the successful candidate will need to work flexibly, creatively and collaboratively.  Depending on organisational demands, this role may likely be extended. The requirement to work a day on the weekend will also be preferable. 

 

Experience/Skills

  • Applicants must hold a Bachelor Degree in Social Work
  • Applicants must have an understanding of the psychosocial issues associated within a hospital setting 
  • Applicants will need to have had experience in working within a hospital setting 

Essential:

It is vital that the successful candidate has experience in working within an Electronic medical record.
