  • Maternity leave replacement, 72 Hours per fortnight

The Department

The Alfred Health HITH unit is a leading provider of acute home based health care in Victoria. The unit currently manages and treats in excess of 50 patients daily.

This dynamic role involves collaborating with multidisciplinary teams across a wide range of medical and surgical specialities within Alfred Health, and with outsourced service providers.

Patients more frequently accessing HITH include Cystic Fibrosis, Transplant, Heart Failure, Burns and Cancer patients, making Alfred Health HITH both a challenging and rewarding place of employment.

The Role

Come and be a part of the proactive and innovative Alfred Health HITH team. You will preferably have recent acute care experience, advanced clinical assessment skills and the ability to work autonomously in the community. You will be motivated, flexible and display strong communication skills. You will be patient centred and committed to ambulatory care. You will understand the importance of establishing a relationship between the patient and yourself in order to achieve the best possible outcomes for your patients. You will be required to demonstrate the core nursing values of patient centred care, trust and respect in every interaction with patients, carers and colleagues.

  • Demonstrated team player.
  • Experience within the community nursing setting desirable.
  • A current Victorian Drivers Licence.
