The Department

 An exciting role exists within the Social work department at Caulfield Hospital for an experienced Social worker in Rehabilitation.  You will have experience in providing strong advocacy with patients and families. 

 The Role

A sound knowledge of the NDIS, ABI service delivery & Aged care/TAC services.  The successful candidate will also possess strong communication skills and be able to have proficient working knowledge of using an Electronic medical record.


  • Promote appropriate standards of professional social work practice and maintain professional relationships within Caulfield Hospital through active liaison with Medical, Nursing, Allied Health staff, other ward and unit staff.
  • Actively participate in special interest groups, departmental staff meetings and team meetings.
  • Participate in research opportunities within the Social Work team including quality improvement, consumer feedback and program evaluation.
  • Participate in annual on-line Performance Reviews and updating of the Performance and Development Plan.
  • To be accountable for own practice through formal supervision and staff appraisal processes and to provide supervision and appraisal for designated others
  • Maintain and update departmental resource files.
  • Share information regarding diagnostic groups, new techniques and trends in intervention, particularly after attending study groups, conferences and seminars.
  • Regularly participate in supervision with professional supervisor to develop and evaluate your own quality of care and professional performance.
  • Work closely with the senior Social Worker to co-ordinate service delivery across social work and Allied Health   at Caulfield Hospital.
  • Contribute to quality improvement activities, eg. Department audits.
  • Alfred Health is committed to providing practitioners with the opportunity to enhance their clinical practice and expertise while meeting the needs of Clinical Units. Social Workers may at times be rotated across Clinical Units with the aim of increasing their range of experience and clinical expertise and to meet the needs of the organisation.


  • Eligibility for membership of Australian Association of Social Workers. Bachelor of Social Work or recognised equivalent.
  • Relevant clinical experience. A demonstrated interest in continued learning opportunities is also desirable. Personal: Ability to work effectively in inter-disciplinary teams and with families and other colleagues, exhibiting excellent communication skills
  • This position may be required to work at any or all campuses of Alfred Health as and when required. Such a requirement may be on-going, a temporary arrangement, on an ad hoc basis or a permanent change of location.
