The Department

The Victorian HIV Service provides a wide range of specialist outreach services and a care coordination service in collaboration with other health care and service providers, hospital staff and community agencies.  Within its statewide mandate the service aims to identify emerging HIV health service needs and work with agencies to address gaps in services, ensure equitable access to HIV care across Victoria and across populations and provision of quality HIV care. 

The position is one of several specialist HIV nurses working in an outreach capacity within a multidisciplinary team to provide high quality HIV nursing care.   The focus of the service is on patients either in the community setting or another health service’s ambulatory/inpatient setting.  This will include; aged care settings, GP practices, hospital settings, custodial settings, supported accommodation services. 

The Role

The role will provide clinical leadership in; developing service responses and provision of best practice HIV services in partnership with agencies, care coordination of patients with complex health and psychosocial needs and leadership in the provision of HIV nursing care.  They will provide education and support to health professionals, health services and care providers to maximise care for people living with HIV (PLHIV).  They will demonstrate a high degree of clinical autonomy and work in partnership with the Infectious Diseases Physician who provides a leading role in HIV medical outreach service development and has responsibility for clinical governance within the Statewide Outward Program. 

As part of a multidisciplinary care coordination team they will provide nursing care coordination to patients with a range of complex health and psychosocial needs with guidance from senior team members.   This will include undertaking expert HIV nursing assessments and development of and implementation of care coordination plans with a range of health and community service providers.   They will provide mentoring and support to junior nursing staff within the team. 

Qualifications and Experience Required

  • Current registration with the Nursing and Midwifery Board of Australia
  • Post graduate qualification in HIV/Infectious Diseases or other relevant field is desirable but not essential
  • Tertiary qualification in nursing or health related field or working toward same desirable
  • Experience in the HIV/ID field or Community Nursing is desirable
  • Strong clinical nursing background and strong professional identity
  • Current Victorian Driver’s license is essential
