The Department

The Alfred Health HITH unit is a leading provider of acute home based health care in Victoria. The unit currently manages and treats in excess of 50 patients daily.

This dynamic role involves collaborating with multidisciplinary teams across a wide range of medical and surgical specialities within Alfred Health, and with outsourced service providers.

Patients access HITH for a range of treatments including for Cystic Fibrosis, Transplant, Heart Failure, Burns, and Cancer, making Alfred Health HITH both a challenging and rewarding place of employment.

The Role

Come and be a part of the proactive and innovative Alfred Health HITH team. You will have recent acute care experience, advanced clinical assessment skills and the ability to work autonomously in the community. You will be motivated, flexible and display strong communication skills. You will be patient centred and committed to care beyond the hospital walls. You will understand the importance of establishing a relationship between the patient, their families and yourself in order to achieve the best possible outcomes for your patients. You will demonstrate the core nursing values of patient centred care, trust and respect in every interaction with patients, carers and colleagues.

Skills and Experience Required

  • Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
  • Demonstrated interpersonal skills
  • Demonstrated organisational skills
  • Demonstrated motivation and enthusiasm
  • Demonstrated team player
  • Experience within the community nursing setting desirable
  • A current Victorian Driver's Licence
