  • Monday to Friday business hours initially, with view to review and adjust according to service demands

The Department

The Mobile Assessment and Treatment Service (MATS), otherwise known as Alfred Health’s Residential In Reach Service (RIR), is a dynamic fast paced clinical service, providing outreaching acute care to older people living in residential care in the Alfred Health catchment, with aim of providing care in place where ever safe and appropriate.    

If successful, you will be joining a dedicated team who are highly respected within Alfred health, and have built a positive profile in our local Aged Care community.

We are looking for someone who is:

The Role

The Residential In Reach (RIR) NP will have the following responsibilities:

  • Expert care: Provide comprehensive assessments, diagnoses, and tailored treatment options directly to patients, whether in-person or through Telehealth
  • Collaborative planning: Work with multidisciplinary teams to create personalised care plans that prioritise patient goals and preferences
  • Empowerment through education: Offer health education and support to patients, families, and care staff, ensuring optimal outcomes and understanding of care processes
  • Community connections: Build strong partnerships with GPs, residential care staff, and community services to manage patient care effectively, reducing avoidable hospital admissions
  • Adapt and lead: Evolve your practice to meet the needs of our service and take an active role in professional development and leadership within the MATS team.

Experience and Qualifications Required 

  • Current endorsement by the Australian Health Practitioner Regulation Agency as a Nurse Practitioner
  • Masters of Nurse Practitioner (or a Masters granted equivalence of qualification by the Australian Health Practitioner Regulation Agency plus an approved Therapeutic Medication Management module) or working towards completion of
  • Evidence of competent utilisation of extensions to advanced nursing practice according to approved Clinical Practice Guidelines in the clinical area
  • Advanced interpersonal, communication (written and verbal) and problem solving skills
  • Knowledge, understanding and experience in research and quality improvement
