About us
The Alfred Care Group - Renal Service incorporates extensive consultative and management program for inpatient, ambulatory and community based renal care. These renal services are provided across the acute inpatient ward at The Alfred, ambulatory and outpatient settings and three dialysis units across the Alfred Health campuses including Caulfield and Sandringham sites.
Services are provided across the continuum of acute and chronic kidney disease, including dialysis (including for acute kidney injury) and transplantation. Dialysis is provided through in-centre, home and satellite dialysis facilities. Renal transplantation includes both deceased and live donor as well as multi-organ transplants. There are strong links with Peninsula Health where a hub and spoke model supports the satellite haemodialysis services at Frankston, Hastings and Rosebud Hospitals.
About the role
The Renal Transplant CNC will be responsible for providing professional support and clinical expertise to people with end stage renal disease who have chosen a kidney transplant care pathway.
The position works closely with the multi disciplinary Renal team to coordinate and streamline transplant assessments; manage the wait list and provide post transplant care for both recipients and donors.
About you
- Current Registration as a Registered Nurse with Nursing and Midwifery Board of Australia via AHPRA.
- Minimum of 5 years experience in Renal nursing including transplantation
- Post graduate qualification in Renal Care preferred/working towards Masters qualification
- Motivated, autonomous with an ability to work collaboratively with departments across many health services and community
- Proven clinical leadership skills and a commitment to quality and safety and research activities
