Department Summary

Hospital Admissions Risk Program Complex Care (HARP CC) is a HIP program and in Alfred Health sits within Alfred Home - Acute and Community (HAC). The role of HARP as described by the Department of Health and Human Services (DHHS) is to “provide hospital substitution and diversion by supporting people in the community, in ambulatory settings, and in people’s homes.”  Alfred Health’s HARP Complex Care Service is comprised of a multidisciplinary workforce working in an interdisciplinary model of care. This position is open to an experienced registered nurse that has a strong interest in chronic and complex holistic health care. This role works within a geographic team with supervision and support from the HIP Complex Care Coordinators, working alongside the social work case managers, HARP nurses, and allied health clinicians.

Position Summary

The HARP CC Case Manager is responsible for providing short-term case management and care coordination to clients who have complex psychosocial issues and chronic health conditions that place them at risk of potentially avoidable hospital presentations. The Case Manager also utilises their nursing education and skills to provide a holistic approach to care including chronic disease management where required. Case Managers work in a multidisciplinary team utilizing an interdisciplinary approach to care. On a day-to-day basis, the role is overseen by the relevant site-based HIP Complex Care Clinicians (HIP CCC). Direct Care, including comprehensive psychosocial assessment and clinical intervention, is delivered within the context of an individual care team for each client. Clinical interventions that require medical management are overseen by the HARP registrar and implemented in collaboration with the Case Manager. The Case Manager holds a caseload of clients and works directly with the client to develop client-centered SMART goals. In collaboration with the multidisciplinary team, the CM develops a holistic approach to achieving these goals set out in the client’s care plan. Clinical interventions and case management strategies are individualised and flexible to ensure effective and efficient service delivery.

Skills and Experience Required

  • Nursing applicants require, four or more years of nursing experience following graduating from a Bachelor of Nursing
  • Allied health applicants require minimum 2 years experience  
  • Experience caring for complex clients in the community or complex discharge planning from an inpatient setting
  • Sound understanding of ACAS, home care packages, My Age Care, NDIS, and other essential primary care stakeholders
  • Knowledge on how to improve patient safety and quality of life through the promotion of health independence.
  • Be able to recognize deterioration and or risk in the community and escalate appropriately
  • A strong interest in caring for people who experience significant challenges in both chronic disease, vulnerability, and psychosocial complexities
