The Department

The Adult and Older Adult Department combines the strengths of Alred Mental and Addiction Health (AMAH) Adult Community Mental Health and Caulfield Aged and Liaison Psychiatry Services. Our mission is to provide comprehensive, person-centred support to adults and older adults through an expansive range of therapeutic, addiction, trauma-informed, and family violence services.

As part of our commitment to the Royal Commission into Victoria’s Mental Health recommendations and the Victorian Mental Health and Wellbeing Act 2022, we’re expanding our offerings to ensure clients benefit from specialist expertise alongside access to innovative, broad-spectrum care options. This includes specialized pathways for substance use disorders, rehabilitation and recovery, physical and mental health integration, and trauma-focused services.

The Role

The newly created CCT Team Leader is a senior clinical leadership role responsible for the day-to-day clinical, operational, and people leadership of the Continuing Care Team. The role supports the safe, timely, and recovery-oriented delivery of assessment, treatment, and ongoing care for adults and older adults, while promoting workforce wellbeing, capability, and performance.

The CCT Team Leader supports the Operations Manager – Continuing Care in the implementation of the Royal Commission into Victoria’s Mental Health System outcomes and the AMAH Transformation Strategy. The role works collaboratively with leaders across the Adult and Older Adult Services (AOAS), including the Team Leader Lived Experience Peer Work and Consultant Psychiatrists, to improve consumer and carer experience, patient flow, and timely access to appropriate services.

Experience and Qualifications Required

• Tertiary qualifications and either current registration with the Nursing & Midwifery Board or Occupational Therapy Board via AHPRA, or eligibility for membership with the Australian Association of Social Workers, preferably with Psychiatric endorsements or equivalent. 
• Post graduate qualifications preferably in mental health or community health.
• Management qualifications or working towards would be highly regarded.
• Note: Social Workers require a satisfactory completion of a Working with Children Check.
• Minimum 7 years' experience in mental health including substantial experience in health service management and a record of delivering high-quality service outcomes.

Other requirements

• Sound knowledge of the Mental Health and Wellbeing Act 2022 and other relevant Legislation and their relevance to this client population.
• Proven ability to provide effective clinical leadership, supervision and day-to-day guidance to multidisciplinary clinicians and lived experience staff, supporting safe practice, accountability and workforce capability.
• Sound knowledge of the associated disabilities that often co-exist with mental health conditions and be able to demonstrate skills in a broad range of psychiatric and psychosocial interventions for consumers referred to CCT.
