The Department

The Victorian HIV Service provides a wide range of specialist outreach services and a care coordination service in collaboration with other health care and service providers, hospital staff and community agencies.  Within its statewide mandate the service aims to identify emerging HIV health service needs and work with agencies to address gaps in services, ensure equitable access to HIV care across Victoria and across populations and provision of quality HIV care. 

The position is one of several specialist HIV nurses working in an outreach capacity within a multidisciplinary team to provide high quality HIV nursing care.   The focus of the service is on patients either in the community setting or another health service’s ambulatory/inpatient setting.  This will include; aged care settings, GP practices, hospital settings, custodial settings, supported accommodation services. 

The Role

One of the services within the Victorian HIV Service is the HIV Outward Program, this is a multidisciplinary team which leads a wide range of specialist outward (Outward from The Alfred, extending across Victoria) services and responses.  The team support people living with HIV (PLHIV) with a range of complex health and psychosocial needs.  In addition, they support marginalised populations and specialty PLHIV populations’ access equitable healthcare, such as provision of specialist HIV support to maternal health services for pregnant women. 

This position works within a small HIV nursing team and more broadly the multidisciplinary team within a medical model of care under the direction of an Infectious Diseases Specialist.  The role will provide nursing assessment, develop, implement and action care plans. Multidisciplinary/multiservice care plans will be developed for PLHIV through engagement with the patient, a range of community services and agencies, under the supervision of a Clinical Nurse Consultant and the Infectious Diseases Physician. 

Qualifications and Experience Required

  • Registration with Nursing and Midwifery Board of Australia
  • Experience in the HIV/ID field or Community Nursing is desirable
  • Strong clinical nursing background and strong professional identity
  • Current Victorian Driver’s license is essential
  • Current and satisfactory police check
  • Current and satisfactory Working With Children Check
