The Mobile Assessment & Treatment Service (MATS) is a community and ambulatory service within the Health of Older People division of Alfred Home, Acute and Community (HAC).
MATS overseen by Geriatricians, is a multidisciplinary team of Medical, Nursing, Nutrition, Pharmacy and administrative staff.
The service provides rapid clinical in-reach to people living in residential aged care facilities [RACFs] and the community who are at risk of presenting to emergency departments for reasons that could be managed outside of the hospital setting. The MATS patient cohorts are the frail and elderly, or those with severe disability in residential settings. Often MATS patients have a range of chronic conditions, and conditions of aging.

MATS is looking for a dynamic and experienced Registered Nurse to join our team. The successful applicant will be motivated to optimise the health experiences of frail elderly patients living in residential care and the community setting.
