  • Part-time role: 24 hours per week (16 hours in Community Rehabilitation Program and 8 hours in ABI Community Rehabilitation Service)

A fantastic opportunity for an experienced Dietitian to join our Community Rehabilitation Program and ABI Community Rehabilitation Service teams.

The Department

Community Rehabilitation Program (CRP)

  • Alfred Health Community Rehabilitation Program provides Home and Centre Based rehabilitation as well as Falls Clinic services.
  • The service's focus is on provision of goal-directed, community based rehabilitation for clients following injury or hospitalisation.
  • The Community Rehabilitation multidisciplinary team comprises physiotherapists, occupational therapists, speech pathologists, dietitians, a neuropsychologist, a clinical psychologist, allied health assistants, rehabilitation medicine doctor, a pharmacist, a registered nurse and social workers.

Acquired Brain Injury Community Rehabilitation Service (ABI Comm Rehab Service)

  • The ABI Community Rehabilitation Service provides a state-wide specialist rehabilitation service to people with moderate to severe ABI. This includes people with traumatic brain injuries, stroke, hypoxic brain injuries and other causes of non-progressive brain injury.
  • The Community Rehabilitation Service provides long term management and support, in partnership with local services. The service focusses on the lifelong, evolving needs of people with an ABI.

The Role

CRP:

  • The overall objective of the position is to provide high quality clinical nutrition and dietetic services to clients referred to the Community Rehabilitation Program. 
  • Treatment programs can be conducted in the client’s home or in the community rehabilitation centre at Caulfield Hospital.  
  • The clinical caseload is varied and may include neurological, orthopaedic, cardiac, spinal, pulmonary and functional decline diagnoses. 
  • The position aims to provide a flexible and responsive service that works to meet each client’s identified rehabilitation goals.

ABI Comm Rehab Service:

  • Counsel patients on nutrition issues and healthy eating habits. 
  • Develop meal plans, taking both cost and clients’ preferences into account. 
  • Evaluate the effects of meal plans and change the plans as needed. 
  • Write reports to document client progress. 

Qualifications and Experience Required

  • Bachelor in Nutrition and Dietetics (or equivalent)
  • Accredited Practicing Dietitian (APD) status
  • Registration with AHPRA
  • Victoria's Driver's License
  • Membership of the Dietitian Association of Australia (DAA)*
  • Working with Children Check and Criminal History Check
  • Registration as an NDIS Provider*
  • NDIS Worker screening check*
  • Completion of online NDIS-training*
  • If providing home enteral nutrition therapy specific experience/ knowledge of home enteral nutrition essential*

Desirable:

  • Experience working with people living with disability   
  • Minimum 3 years' experience
  • Experience in rehabilitation or aged care
  • Demonstrated ability to work effectively in an interdisciplinary team
  • Demonstrated understanding and commitment to principles of continuous quality improvement
  • Demonstrated ability to evaluate and modify service delivery in accordance with evidence-based practice
  • Well-developed communication and interpersonal skills to promote teamwork
  • Strong theoretical knowledge base and experience in the areas of orthopaedics, neurology and gerontology 
  • Demonstrated participation in professional development relevant to the position

Highly desirable:

  • Recent experience in community-based rehabilitation.
  • Experience in a grade 2 Dietitian role in a public health service.
