About the team

Alfred Mental & Addiction Health (AMAH) is responsible for the operation of services, which focus on people with a severe mental illness residing in the Inner South Eastern area of Melbourne.

The single most important goal of AMAH is to create an environment that facilitates clinical recovery, supports individual recovery efforts and strives to minimise service system barriers to the recovery process.

MOBILE AGED PSYCHIATRY SERVICE (MAPS)

MAPS provides care to patients living at home, including within residential aged care facilities.

MAPS is an interdisciplinary team which undertakes assessment treatment and case management of older persons over the age of 65 years with mental health problems living in the Inner South East region of Melbourne.

The role

As one of the Case Managers, you will be responsible for a range of clinical mental health services including triage/intake, mental state examinations, risk assessments, care planning, treatment, case management and discharge planning for persons generally over the age of 65.

You will act as a resource person to other staff in clinical problem solving and is also responsible for the promotion of Aged Persons Mental Health Services, supporting the Team Leader/Manager in orientation and development of new staff. You will also be expected to carry a portfolio as designated by the Team Leader/Manager.

Qualifications & Experience Required

Essential 

  • Approved tertiary qualifications and eligibility for membership with relevant registering body (Nursing and Midwifery Board / Occupational Therapy Board via AHPRA, or eligibility with AASW)
  • Minimum two years’ experience in mental health settings - desirable
  • Have, or working towards, Post graduate qualification(s) in Mental Health - desirable
