Alfred Health is a leader in health care delivery, improvement, research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, from ambulatory to inpatient and home and community-based services. On 1 January 2026, Alfred Health will come together with Kooweerup Regional Health Service, Peninsula Health, Bass Coast Health and Gippsland Southern Health Service to create Bayside Health, a newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians. The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.

The Department
Alfred Health Carer Services (Carer Services) reports to the Deputy Director of Operations - Community Integration, which is part of the Rehabilitation Aged and Community Care program. Carer Services supports carers, the person being cared for and care relationships in a client-centred, timely and responsive manner.
The Role
This position contributes to the development and delivery of client-facing programs within Carer Services. This role offers the chance to support clients from a wide range of cultural communities. You will play an important part in providing culturally safe, respectful and person-centred care.
Carer Services provide service to people who are 65 years and over (50 and over for Aboriginal and Torre Straight Islanders), people with dementia and their carers

Qualifications and Experience Required
- Tertiary qualifications in Health, Behavioural Sciences, Social Sciences, Community Development or Welfare
- Minimum 3 years experience and knowledge of aged and dementia practice areas
- Demonstrates understanding of issues facing carers and people from diverse communities and available services
- Experience in casework and care coordination, including needs/risk assessment and support planning
- Ability to provide support, identify issues, escalate appropriately and operate within scope of practice
