  • Permanent Position   
  • Full Time, 80 hours per fortnight + ADO

The Department

The Social Work Department operates in accordance with the philosophy and goals of The Alfred.  A focus on the quality of life and participation in care is a primary goal of the Department. Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services that increase the quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home. 

 

The Role 

We are seeking a Grade 1 Social Worker to join our Social Work team at Alfred Health. You will have relevant experience and be able to work within a strong multidisciplinary team.  To be successful, you will need to have a good understanding of hospital social work,  have a good knowledge of social work theories and be able to apply them to hospital practice. It will be essential to have a sound understanding of the key elements of supporting a complex discharge plan within a hospital setting.  An understanding of services and supports in particular an understanding of the Transition Care Program, My Aged Care and the NDIS will be beneficial for this role.

The successful candidate will need excellent communication skills to collaborate with patients, families, other disciplines, and community stakeholders.  Healthcare is a fast-paced environment and requires timely and quality discharge planning and the successful candidate will need to work flexibly, creatively and collaboratively. This role will work in the Neurology/Stroke area of The Alfred Hospital providing support to patients and families within this clinical unit.  The need to provide support across the Social work department during periods of high volume will also be required.  This role will provide you with the ability for career development and will allow you to be part of a robust training program which will involve professional development to upskill you to further develop your Social work skills.  The requirement to work one weekend day per month will also be preferable for this role. 

 

Qualifications/Skills Required 

  • Applicants must hold a Bachelor's Degree in Social Work
  • Applicants must have an understanding of the psychosocial issues associated with a hospital setting 
  • Applicants must have a solid understanding of the services and support available to patients and families

This role will require you to work a weekend day once a month.
