The Department

Heart and Lung Transplant Services – Nursing The transplant services cross both cardiac and respiratory services with the Heart & Lung Transplant service provision providing pre and post- transplant care.

 

Position Summary

The Transplant Ambulatory Care nurse provides holistic care to the patient and their carer/s throughout the transplant journey from pre-transplantation through to ambulatory care post transplantation. They will have the clinical expertise to assess, plan, facilitate, implement and monitor the care of patients in the transplant unit. The Transplant Ambulatory Care nurse works within a multidisciplinary team, to promote autonomy and independence of the patient to develop the capacity to manage their own health as far as possible. The Transplant Ambulatory Care nurse role has a strong focus on education and establishing and maintaining therapeutic relationships with the patient and their carer/s. Effective communication with the patient, carer/s and the health care team is integral to this role. The Transplant Ambulatory Care nurse will also work as part of a team in the process of recipient Transplant Coordination; that is, being able to assist in the process pertaining to heart and lung transplantation within and out of hours. The Transplant Ambulatory Care nurse also works flexibility to support the team as required.

 

Qualifications/Experience Required

Essential

  • Registered as a Registered Nurse with Nursing and Midwifery Board of Australia.
  • Extensive acute clinical nursing experience – Respiratory, Cardiothoracic and Lung +/- Heart transplantation experience an advantage.
  • Working with children certificate

Desirable

  • Understanding of ambulatory care models.
  • Relevant post-graduate diploma or certificate is preferable
  • Recent leadership position.
