Position Summary

Alfred Health is transforming approaches to healthcare delivery through our Interconnected Care Program. We aim to optimise patient care coordination, improve patient access, maximise resource utilisation, and enhance the overall healthcare experience for both patients and employees through innovative technologies, process enhancements, and new care models.These new models of care will ensure that the quality of care received at home is as good as, if not better than, that provided in hospital settings. 

 

We're looking for a dynamic and passionate Interconnected Care Program Manager to drive the operationalisation of interconnected care models and support transformational system-wide change. This position sits within the Alfred Health Operations team and is crucial in shaping patient-centred care pathways both within and outside the hospital setting. The role will drive the implementation of advanced virtual care solutions for remote care management and clinical monitoring, remote consultancy and support the scaling of care outside hospital walls. Your work will directly enhance healthcare delivery, extending the reach of quality care beyond traditional hospital settings.

 

Key Responsibilities

  • Lead the design and operationalisation of acute care pathways, integrating remote monitoring, remote consultancy, and virtual care technologies.
  • Collaborate with a multidisciplinary project team to develop and implement innovative care models prioritising patient safety and high-quality care.
  • Engage with a broad spectrum of stakeholders, including nursing staff, other healthcare disciplines, and external vendors, to ensure the successful delivery of the Interconnected Care project.
  • Drive the development of patient escalation processes and safety protocols, focusing on operational excellence and patient-centred outcomes.
