An exciting opportunity exists for an experienced and motivated healthcare professional to join the Transition Care Program (TCP) at Caulfield Hospital as a Case Manager.

Working within a multidisciplinary team, you will play a vital role in supporting older people as they transition from hospital to home or into other supportive care arrangements. This rewarding role offers the opportunity to make a meaningful difference by helping clients maximise their independence, achieve their goals, and access the services they need for a successful transition to community living or into a supportive care environment.

About the Transition Care Program

The Transition Care Program (TCP) provides short-term, goal-oriented reablement and therapy services for older people leaving hospital.

The program supports clients to:

  • Regain independence and optimise their functional ability following a hospital admission.
  • Access restorative care and support services.
  • Explore and establish appropriate long-term care arrangements.
  • Connect with ongoing community or residential support services when required.

TCP provides up to 12 weeks of support, delivered either:

  • In the client's home, or
  • In a residential aged care setting.

About the Role

As a TCP Case Manager, you will coordinate and manage a comprehensive package of care and services for clients enrolled in the program. Working closely with associate providers, health professionals, clients, families, and community organisations, you will ensure clients receive high-quality, coordinated care while progressing towards their discharge goals.

Key responsibilities include:

  • Coordinating admissions into the Transition Care Program.
  • Completing assessments and developing individualised Care and Service Plans in partnership with clients.
  • Coordinating community-based services and supports to meet client goals.
  • Monitoring client progress and supporting achievement of reablement outcomes.
  • Facilitating effective discharge planning and transitions to long-term support arrangements.
  • Supporting clinical care coordination and client management where required.
  • Liaising with hospitals, specialist clinics, general practitioners, and community service providers to ensure seamless care delivery.
  • Maintaining accurate documentation and ensuring compliance with program requirements.
