The Department

The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on the quality of life and participation in care is a primary goal of the Department.
Under the direction of the Manager of the Social Work Department, the following services are provided:

  • Social Work

 

Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services that increase the quality of life and participation in care. Social Work also assists patients and their families transition smoothly from hospital to home.

 

The Role

Exciting Full-Time Opportunity for an Experienced Gr 3 Social Worker at Alfred Hospital – Fixed Term Until December 2025

Join a dedicated team in the Trauma Social Work Service at Alfred Hospital and make a real difference in the lives of patients and their families. We’re seeking an experienced Social Worker (Grade 3) with advanced skills and hospital experience to provide crucial support in the Trauma Service, offering client-centred care through counselling, advocacy, and guidance throughout the patient journey.  As part of a dynamic, interdisciplinary team, you’ll play a key role in conducting comprehensive psychosocial assessments, coordinating safe discharge plans, and ensuring continuity of care. Your expertise will help reduce hospital stays, prevent readmissions, and improve patient outcomes while working closely with families, external stakeholders, and the wider medical team.

What we’re looking for:

  • Proven experience in psychosocial assessments and advocacy

  • Strong conflict resolution skills and the ability to manage sensitive issues

  • Hospital setting experience and understanding of electronic medical records

  • Strong understanding of working with patients in a Trauma and Acute health setting

  • Leadership experience 

  • Availability to work one weekend day per month

  • At least 7years clinical experience in Social work is essential for this role
