Department

Hospital Admissions Risk Program Complex Care (HARP CC) is Health Independence Program (HIP) and in Alfred Health sits within HAC. The role of HARP as described by the Department of Health and Human Services (DHHS) is to “provide hospital substitution and diversion by supporting people in the community, in ambulatory settings, and in people’s homes.” Alfred Health’s HARP Complex Care Service is comprised of a multidisciplinary workforce working in an interdisciplinary model of care. The HARP service structure consists of four geographically based teams, a centralised team made up of discipline-specific clinicians and a telephone coaching service for nutrition support to clients with cardiac conditions.

Position Summary

The Allied Health Assistant works as part of the multidisciplinary team within the Central Team. The Allied Health Assistant is aligned to the Central Team, and therefore functions across the HARP service, supporting the geographic teams, senior social worker and the primary care clinicians. The AHA works in a flexible and adaptive manner as the HARP program continues to evolve. Responsibilities include supporting the follow up and care coordination of vulnerable adults under the guidance of the senior social worker. Supporting the geographic teams with My Aged Care referrals and administrative follow up.

Qualifications & Experience Required

  • Completion of Certificate III (Allied Health Assistance) from a registered training organisation or its equivalent.
  • Demonstrated experience in a hospital or healthcare setting
  • Demonstrated attributes of empathy and client centred focus
  • Effective communication skills, both written and oral
  • Demonstrated capacity to work in an interdisciplinary team
