Would you like to work with a progressive, supportive social work department and make a real difference with the patients and families you work with?

A full time Grade 2 Social Work position is available at Sandringham Hospital.

We are looking for an experienced, skilled and enthusiastic hospital social worker who can confidently work across the acute care of the elderly, and be available to provide social work leave cover in general medicine, emergency and maternity services. The successful applicant will have solid clinical skills and proven competencies in psychosocial assessments, counselling, and discharge planning and the flexibility in managing the competing demands and changing practices of a busy community hospital.

The Department

The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on the quality of life and participation in care is a primary goal of the Department.

Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services that increase the quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home. This role is an exciting role at Sandringham Hospital developed to meet the needs of the growing inpatient service.

You will be part of a dynamic Social Work team in providing social work inpatient services across the Hospital. The position is part of a strong allied health cohort that works collaboratively in meeting the complex demands and discharge concerns of this client group.

You will be part of a larger Social Work Department within Alfred Health Care Group and have access to various professional development and supervision opportunities.

The Role

You will become part of a dynamic and supportive department that is committed to excellence and innovation. We provide skilled supervision and ongoing professional development. This role will work with acute inpatients with a strong focus on multidisciplinary teamwork. You will be involved in the discharge planning and support for patients and families. This role would suit someone who is well organised, and able to work within a fast-paced unit with demonstrated skills in psychosocial assessments, problem-solving, communication and hospital discharge planning. You will be a dynamic, innovative and flexible practitioner with a good knowledge of resources, residential care processes and referral networks. Experience in dealing with complex discharge planning will be essential.

This position may require one to work at any or all campuses of Alfred Health Care Group when required. Such a requirement may be ongoing, a temporary arrangement, on an ad hoc basis or a permanent change of location. You will be required to work one weekend day per month.

Skills and Qualifications Required 

Essential

  • Eligibility for membership of Australian Association of Social Workers. Bachelor of Social Work, or recognised equivalent
  • Required to perform work that requires special knowledge and depth of experience in any one or more of the following:
    • Family Violence/ Child Protection/ NDIS/VCAT Guardianship List
  • Comprehensive knowledge and counselling skills related to chronic illness, illness behaviour and adjustment to illness, loss and grief
  • Knowledge of current Legislation and Policy in relation to Social Work in a hospital setting
  • Experience with student and staff supervision preferred
  • Previous aged care and rehabilitation experience is desired
