  • Fixed term (12 month contract)

The Department

The newly integrated Adult and Older Adult Service combines AMAH Adult Community Mental Health and Caulfield Aged and Liaison Psychiatry Services to provide comprehensive, person-centred support for adults and older adults. We offer a wide range of therapeutic, addiction, trauma-informed, and family violence services.

In line with the Royal Commission into Victoria’s Mental Health recommendations and the Victorian Mental Health and Wellbeing Act 2022, we are expanding our services to provide specialized expertise and broad-spectrum care, including pathways for substance use, rehabilitation, recovery, and trauma-focused care.

Our approach focuses on integrated treatment, education, peer support, care planning, and navigation, supporting individuals at every stage of their recovery journey.

You’ll have the opportunity to work in areas such as:

  • Navigations and Continuing Care: Brief Intervention, Access and Intake, CMHT, Physical Health, and Family Services
  • Adult Rehabilitation and Recovery: MSTS, CCU, PARC, HOPS, and SECU
  • Aged/Older Adult Mental Health: MAPS, Caulfield Hospital Community Liaison, and inpatient units
  • Wellbeing and Therapeutic Services: Psychological Trauma and Capacity Building initiatives

The Role
A Community Transition to speciality practice Mental Health Nursing position has been established to provide opportunities for nurses moving from a psychiatric inpatient unit into a community mental health setting.  It is a twelve-month structured and supported program enabling a mental health nurse to build on knowledge, skills and confidence relevant to community mental health nursing.  A transition to speciality practice nurse will work within a multidisciplinary team and in partnership with individuals/families/carers/networks working towards recovery goals.  This program offers protected time with a caped case load to enable work with a mentor, ongoing education and clinical supervision. 
Skills
The 12 month program will focus on development of core skill:

  • Specialist community mental health nursing skills
  • Advanced assessment skills
  • Family consultation
  • Psychosocial and therapeutic interventions
