About the role

An exciting opportunity is available for an experienced hospital-based Social Worker to join Alfred Health's Social Work Department. This dynamic and supportive team values excellence, innovation, skilled supervision, and ongoing professional development.

The role involves providing leadership and support to a small team within the Special medicine stream.  This is a new role that will aim to provide social work services to patients and families in Renal Services at The Alfred, with a particular emphasis on providing support to the satellite dialysis services at Caulfield and Sandringham, and Renal outpatients. Responsibilities include delivering psychosocial support to patients with Chronic Kidney Disease, contributing to quality improvement initiatives, and supervising junior staff and students. 

The ability to lead a team and have relevant leadership experience will be crucial for this role. A high level knowledge of NDIS, TCP, aged care processes, community resources, and referral networks is vital. Applicants must be eligible for AASW membership. This position therefore requires a social worker with demonstrated senior clinical experience, strong leadership capability, and proven competence in staff supervision, student education, and complex clinical practice across multiple service domains, particularly within a high-demand acute health setting. 

This position offers the opportunity to collaborate across various teams and locations, including Alfred Dialysis Satellite Units, home therapies, and transplant services.  This role will require the successful candidate to work across all sites, speficially providing support to satellite sites in Caulfield and Sandringham hospitals. 

About you  

  • Approved degree in Social Work and evidence of eligibility for membership of the Australian Association of Social Workers (AASW).
  • Advanced social work skills 
  • A knowledge of Nephrology services and the psychosocial impact that this area has on this patient population
  • Demonstrated experience in the area of General Medicine and an understanding of this diverse patient population
  • A working understanding of the NDIS and current processes around this service
  • Experience in the ability to be a team player within the larger clinical service team of Renal/General Medicine and demonstrated experience in supporting the leadership within a team and working together with Team Leaders to promote and assist in the leadership of the social work service.
  • Demonstrated ability to manage both an inpatient and outpatient caseload
  • Will need a current Working with Children’s check to undertake this role.
  • Desirable  Post graduate qualifications in Social Work or a related field.
