The Department

The Alfred Health Social Work Department operates within the values, philosophy, and goals of The Alfred, with a strong commitment to enhancing patients' quality of life and participation in care.

Our Social Workers play a vital role in identifying and addressing the psychosocial impacts of illness and hospitalisation for patients and their families. Through comprehensive assessment, counselling, advocacy, and care coordination, we support patients throughout their healthcare journey and facilitate safe and effective transitions from hospital to home and community-based services.

The Role

The Grade 2 Social Worker, Cancer Outpatient Services, is responsible for delivering advanced social work services to patients and their families, carers, and significant others receiving care through Alfred Cancer Services, including patients undergoing bone marrow and stem cell transplantation.

Working across both the Bayside Health -Alfred Care Group Social Work Department and the Paula Fox Melanoma and Cancer Centre, the successful candidate will provide high-quality psychosocial assessment, counselling, advocacy, and care coordination. The role requires close collaboration with multidisciplinary teams and consultation with colleagues across a range of disciplines to support complex patient care needs. The role will also provide specialised psychosocial support to patients undergoing bone marrow and stem cell transplantation, a cohort with complex medical, practical and emotional needs. This includes supporting patients and families throughout assessment, treatment and recovery, addressing adjustment to illness, caregiving challenges, accommodation and financial concerns, and facilitating access to appropriate community and support services.

 Key Responsibilities

• Conducting comprehensive psychosocial assessments
• Providing advanced counselling, including adjustment and supportive counselling
• Supporting patients and families undergoing bone marrow and stem cell transplantation throughout their treatment journey
• Facilitating access to relevant cancer, transplant, community, and support services
• Supporting patients and families to navigate complex health and social systems
• Providing specialist social work expertise in areas such as Family Violence, Child Protection, and NDIS processes and supports
• Contributing to continuity of care across hospital, outpatient, and community settings
• Maintaining accurate documentation within the electronic medical record
• Be able to provide social work support to the Social Work Department across Alfred Care Group during periods of critical demand, including times of significant workforce shortages and high service activity, such as departmental triage days and end-of-year public holiday periods

Skills and Qualifications:

Essential

• Bachelor of Social Work or recognised equivalent qualification
• Eligibility for membership with the Australian Association of Social Workers (AASW)
• Demonstrated ability to work effectively within a multidisciplinary team and with patients and families
• Well-developed initiative, communication, and facilitation skills
• Knowledge of current legislation, standards, and policies relevant to social work practice in a healthcare setting

Desirable

• Postgraduate qualifications, or working towards postgraduate study, in Social Work or a related field
• Experience working with people affected by cancer
• Hospital-based social work experience
• Current Victorian Driver Licence
• Strong information technology and electronic medical record skills
