POSITION:

The Dietitian role is full-time working across two Services at Alfred Health: Hospital Admission Risk Program (48 hours per fortnight), and the Caulfield Community Health Service (32 hours per fortnight). 

DEPARTMENTS:

Hospital Admission Risk Program (HARP)

HARP Complex Care is a service within the range of Health Independence Programs (HIP), and in Alfred Health it sits within the Home Acute and Community Stream.  The HARP dietitian works within a multidisciplinary team to deliver holistic healthcare to people at risk of potentially preventable hospital attendance. This valuable role contributes to the wellbeing of patients by delivering individualised nutritional advice, intervention, referral, or ongoing monitoring of individuals as required.  Caring for a diverse patient cohort, the HARP dietitian supports people experiencing comorbidities by addressing the nutritional impacts that chronic conditions can impose. The HARP dietitian works across the HARP service, including the HARP complex care teams, and when required in the mobile assessment and treatment service (MATS), providing a valuable and much-needed resource.

Caulfield Community Health Service (CCHS)

The Caulfield Community Health Service (CCHS) provides a range of clinical and health promotion services for people of all ages who live, work or study within the local community. These multi-disciplinary services are provided at home, in the centre or community, or via telehealth. CCHS operates from the Caulfield Hospital site.  The Dietitian role sits in one of the adult health multidisciplinary teams and will be responsible for providing client centred nutrition assessment and intervention to clients from a wide range of diagnostic groups.

QUALIFICATIONS AND EXPERIENCE:

The HARP and CCHS service are seeking an experienced dietitian with a passion for community-based clinical practice who will:

  • Provide comprehensive holistic assessments, and time limited intervention to clients from a range of diagnostic groups
  • Develop and implement collaborative, multidisciplinary, patient-centred care plans
  • Work with multi-disciplinary team members to facilitate co-ordinated client care
  • Be self-motivated and able to work well, both independently and collaboratively with clients, their significant others, and internal and external stakeholders
  • Have a strong interest in caring for people who experience significant challenges in both chronic disease, vulnerability, and psychosocial complexities

Essential

  • Tertiary qualifications in Nutrition and Dietetics or equivalent
  • Eligibility for DAA membership · Eligibility to be a Certified Accredited Practising Dietitian
  • Demonstrated dietetic clinical experience relevant to the community health sector
  • Demonstrated ability to work flexibly with internal and external services
  • Current Victorian driver’s licence
