The HIV Service 

The HIV Service are national leaders in HIV clinical care and provide leadership into HIV policy development relating to clinical care.  The Victorian HIV Service have numerous in hospital, outpatient and community services available to The HIV affected community of metropolitan Melbourne and throughout rural and regional Victoria.

The Role

The Social Worker is responsible for the provision of Social Work services to patients living with HIV and their families/significant others as part of The Alfred Statewide HIV Service.  The role works across the inpatient and outpatient hospital setting and within the community to provide a Social Work service to people living with HIV. This role may also provide inpatient Social Work to general ID (Infectious Diseases) inpatients admitted to the Alfred.  In consultation with the Team Leader and Senior Clinician the role includes providing a service to HIV Social Work outreach clinics in the community and other health care settings. This position includes a component of community outreach.

This position is part of the HIV Outward Program providing care and support to people living with HIV who may have multiple complex care needs. The HIV Outward Team is a multiple disciplinary team and the Social Worker will work in collaboration with Nursing, Medical and Mental Health Clinicians. The Social Worker will provide comprehensive psychosocial assessments and reviews through the use of clinical reasoning, to formulate and facilitate timely quality care plans.  The Social Worker will provide counselling to those newly diagnosed with HIV, those with adjustment to illness or changes in care needs, and for other issues including HIV stigma. A patient directed approach is central to how the dynamic multi-disciplinary, team work with the patient.  The development and maintenance of community linkages and partnerships particularity in the HIV sector are critically important in this position.  The Social Worker will be required to consult with the HIV Social Work Team Leader, HIV Senior Social Workers and staff from other disciplines on issues relevant to patient care and provide advanced counselling and case work expertise.  The Social Worker may provide inpatient social work to inpatients living with HIV to provide continuity of care and ensure optimal patient care and contribute to ensuring services and discharge planning for the patient.  This inpatient work will involve building close collaborations and open communication channels between members of all of these teams/units.  

Skills

  • Advanced social work skills and experience as evidenced by preferably 3+ years post qualifying experience and a demonstrated commitment to ongoing professional development.
  • Demonstrated experience in the area of health and an understanding of this diverse patient population and the issues impacting their lives.  A knowledge hospital admission and discharge planning process.
  • The ability to work within a fast paced acute OR outpatient hospital environment and crisis responsive community context.
  • An understanding of HIV stigma, LGBTIQ, loneliness/isolation, CALD/ first nations populations, homelessness, family violence, financial vulnerability, NDIS, aged care
  • A good understanding of adjustment to illness and counselling support is vital for this role
  • Demonstrated experience in social work student supervision and an interest and commitment in the professional supervision of staff.
  • Ability to develop and lead a portfolio or clinic or program within the team
  • A good working knowledge of hospital and community settings and systems is crucial to this role as is a good understanding of working with an Electronic medical record
