Department
General Medicine Hospital in the Home (Gen Med HITH) works closely with the acute inpatient wards and the emergency departments, and collaborates closely with the Hospital Admission Risk Program (HARP) and other community based programs.
Position Summary
Gen Med HITH is seeking expressions of interest from experienced nurses for the role of clinical coordinator. This is an exciting opportunity to expand your clinical leadership skills, and experience a new way caring for patients with acute general medical conditions in the community setting. This service is opening opportunities to patients that would normally be cared for in a hospital bed. Instead they receive their care in the comfort of their home, with the support of nurses, allied health and medical team members. The clinical coordinator is a key role in the multidisciplinary team, facilitating patient access to the service, ensuring that patient care is well-coordinated, and ensuring that patients have robust follow up plans following their acute therapy. The Gen Med HITH coordinator role offers an excellent opportunity to stretch your learning about clinical leadership, and community-based acute care while working in a friendly and supportive team.
Qualifications and Experience Required
- Registered as a Nurse with the Nursing and Midwifery Board of Australia
- Experience in both acute and or ambulatory/community setting is desirable
- A current Victorian Drivers Licence
