Caulfield Hospital provides state-wide rehabilitation services to patients with Spinal injuries, Acquired Brain Injury, Neurological conditions and Amputees.

The Role

This is an exciting opportunity for a motivated and skilled Grade 2 Social Worker to join Bayside Health Alfred Care Group, Rehabilitation and Aged Care service at Caulfield Hospital. Working within a highly regarded multidisciplinary team, you will play a pivotal role in supporting patients and families through complex health journeys, promoting recovery, maximising independence, and facilitating safe and sustainable discharge outcomes.

As a valued member of the Social Work Department, you will provide high-quality psychosocial assessment, intervention, adjustment counselling, advocacy and care coordination for patients with complex rehabilitation and aged care needs. The role offers a diverse and rewarding caseload, requiring advanced clinical reasoning and the ability to navigate complex psychosocial, family, legal, financial and discharge planning issues.

You will work collaboratively with medical, nursing and allied health colleagues to deliver integrated, patient-centred care, contributing your specialist social work expertise to support optimal rehabilitation outcomes. The position requires excellent communication, consultation and negotiation skills, with the ability to build strong therapeutic relationships and engage effectively with patients, families, carers and key stakeholders across the continuum of care.

The successful applicant will provide expert social work support and consultation on complex matters, contributing to multidisciplinary care planning while also supporting colleagues through the sharing of social work knowledge and expertise. The role requires a proactive, flexible and solutions-focused clinician who thrives in a dynamic healthcare environment and can confidently respond to evolving patient and service needs.

While the position is primarily based within Rehabilitation Services, the successful candidate may also have opportunities to contribute to Aged Care units across Caulfield Hospital, further enhancing the breadth and diversity of their clinical experience.
