The Department

The Mobile Assessment and Treatment Service (MATS), is a dynamic fast paced clinical service, providing outreaching acute care to older people living in residential care. 

If you are successful, you will be joining a dedicated team who are highly respected within Alfred health, and have built a positive profile in our local Aged Care community space.

We are looking for an experienced, highly organised and passionate Nurse to join our dynamic team in a new liaison role.  You will work within MATS, and across the following AH teams, including Alfred & Sandringham Emergency Departments (ED), and frequent In-patient Unit referrers (ie: General Medicine (GMU) & Orthopaedic Units) to maximise opportunities for aged care residents to access Residential In reach (RIR) or MATS support when needed.

The successful applicant will be self driven and motivated to optimise the health experiences of frail elderly patients living in the residential care setting. 

The Role

The MATS Liaison Nurse will:

  • Identify patients who have presented to AH setting from Residential Aged Care Facilities (RACF’s), and subsequently support the management of those patients to receive care in a timely manner, by the most appropriate resource and in the best environment.  By doing so, they will demonstrate reduction in avoidable hospital admissions, and assist with earlier discharge of patients back to their RACF.  Along with post hospital support they will ultimately impact the likelihood of re-presentation to AH hospitals in future
  • Provide clinical assessment and risk screening of these patients, and in consultation with the MATS Clinical team including Geriatricians, confirm appropriateness for discharge with MATS
  • Liaise with the ED & In-patient treating teams to provide care planning and discharge coordination that is bespoke to that patient
  • Monitor those patients that are admitted to in-patient teams, and support safe and timely discharge when clinically appropriate under the supervision of the MATS Geriatrician and leadership team
  • Provide clinical review post discharge in the community where appropriate
  • Provide informal learning and development opportunities to ED and in-patient unit clinicians about MATS and what it can provide
  • Interrogate ED data to identify trends or patterns of frequent referrers into ED, develop strategies in collaboration with those facilities in an attempt to reduce future avoidable presentations 
  • Work closely with community stakeholders such as, GP’s, residential care staff, Ambulance Victoria, Victorian Virtual ED (VVED) and other community services, and various specialties across Alfred Health to manage patients in the community thus preventing avoidable admissions to hospital

Experience and Qualifications Required 

  • Ability to communicate effectively, and foster new and maintain ongoing positive working relationships across different hospital departments, as well as with our community stakeholders (ie: RACF staff, GP’s, AV Paramedics & VVED clinicians)
  • Strong analytical, conceptual and writing skills and ability to prepare concise and timely briefs and reports
  • Ability to source and utilise a range of health data to inform project and operational planning
  • RN with extensive clinical experience (minimum 5 years) in acute, general medical and geriatric nursing
  • Nursing registration with AHPRA and Post graduate study desirable
  • Experience in working in projects and change processes desirable
