The Department
Home, Acute and Community (HAC) is a clinical program across Alfred Health. The program plays an integral role within Victoria’s health care system, supporting people to maximise their health, independence and functioning, and minimise long-term care needs. The Home, Acute and Community program oversees a diverse group of services promoting effective seamless care, from acute care back into the community. These services are provided in people’s homes, community centres and in hospital when indicated.
The Role
Reporting to the Deputy Director of Operations - Acute and Integrated Care stream, the Hospital Admission Risk Program (HARP) Manager position will lead the HARP team in monitoring the achievement of clinical and non-clinical performance, providing a service structure that aims to improve the capacity of people living with chronic and complex health and psychosocial conditions. The HARP Manager will work with other HAC programs, the Health Service Partnership and external stakeholders in developing innovative, integrated and scalable models of care, working towards Alfred Health’s strategic goal of providing care outside hospital walls.
Qualifications and Experience Required
• Relevant allied health or nursing qualifications.
• Current registration with the Australian Health Practitioners Regulation Agency (AHPRA) or eligible for membership with relevant professional body, with adherence to continuing professional development requirements.
• Masters level or higher qualification or working towards in health services management/administration/public health or relevant clinical field
• Demonstrated experience in the management of health care teams.
• Highly developed change management skills
• Experience in project management and proven ability to lead projects.
• Experience managing and providing leadership to quality and safety projects.
• Excellent written and verbal communication skills.
• Well-developed negotiation, problem solving and conflict resolution skills.
• Understanding of the health care sector and the Health Independence Programs strategic direction.
• Demonstrated commitment to continuing professional development.
• Current and satisfactory Police Check.
• Recent extensive clinical experience desirable.
