The Integrated Care Team (ICT) is an outward-facing General Medicine Community Team, established to bring together a team of medical, nursing and pharmacy clinicians to provide non-admitted acute care to patients in the community. Alfred Health ICT sits within the Alfred Home - Acute and Community (HAC) and reports through the Community Integrated Stream. ICT seeks to improve the lives of our patients and their carers by:

  • Extending the reach of Alfred Health care into the home
  • Integrating patient care across hospital and the community 
  • Collaborating with Primary care and improving pathways for escalation  
  • Optimising patient access to timely quality care 


The Integrated Care Nurse is responsible for providing direct nursing care, client education, and participating in care coordination for clients being treated by the Integrated Care Team (ICT) within General Medicine. As the Integrated Care Nurse, you understand the importance of building relationships not only between yourself and the client, but also the direct medical team, primary care providers and the wider multidisciplinary team, in order to obtain the best outcome for the client.

The ICT Nurse will facilitate an interdisciplinary care approach in the following settings

  1. Home care- Home visits to provide direct nursing care, both individually or in collaboration with ICT team. This may also take the form of Telehealth or phone reviews and incorporate remote patient monitoring.
  2. Centre based care- Assessment of health care needs and nursing interventions provided in a HAC Hub or treatment room.

Overall the ICT nurse provides clinical care to community patients. The nurse provides consistency to the team and is a resource to rotational doctors, providing knowledge on community services, current patients. The nurse ensures the provision of high-quality, culturally sensitive patient care in partnership with patients, their significant others and other members of the multidisciplinary care team.

Qualifications and Experience


  • Bachelor of Nursing.
  • Current professional registration with AHPRA
  • Four years or more nursing experience.
  • Clinical experience in Emergency Medicine, Acute Care and/or General Medicine (? Desirable).
  • Aptitude for delivering client education using a motivational approach.
  • Demonstrated ability to work flexibly with internal and external services.
  • A high standard of written and verbal communication skills
  • Demonstrated interpersonal skills
  • Commitment to continuing professional development.
  • Current Victorian Driver’s Licence            


  • Post-Graduate studies in any of Critical Care, Chronic Disease, Public Health
  • Experience with Chronic Respiratory disease management and Heart Failure
  • Clinical experience working within the community
