About the Department
The CDAMS Clinic sits within the Home, Acute & Community program, a clinical program across Alfred Health. The HAC program plays an integral role within Victoria’s health care system, supporting people to maximise their independence and functioning, and minimise long-term healthcare needs. HAC oversees a diverse group of services promoting effective seamless care, from acute care back into the community. These services are provided in hospital, community based settings and in people’s homes.
The Cognitive Dementia and Memory Service (CDAMS) is an outpatient Health Independence Program (HIP) funded by the Victorian Department of Health.
CDAMS provides a specialist multidisciplinary assessment, referral, education and advisory service for people experiencing memory loss, confusion or other cognitive changes, and for their carers. There is a CDAMS in each health region of the state, each operating under a consistent state-wide model of practice. The Alfred Health CDAMS, services the inner south region, covering the cities of Port Phillip, Stonnington and Glen Eira.
About the Role
Applications are invited for the position of Visiting Medical Officer in CDAMS at Alfred Health, for 7 hours per fortnight located at Caulfield Hospital. Those from various specialities, with experience in CDAMS are encourage to apply.
Qualifications and Skills Required
Our preferred candidate must possess the following experience and attributes:
- Be legally qualified practitioners, registered or registrable with the Australian Health Practitioner Regulation Agency
- Fellow of the relevant college
- Demonstrable relevant knowledge and experience in CDAMS
- Meeting Alfred Health requirements for credentialing and scope of practice in CDAMS
- Outstanding communication skills, including well-developed conflict resolution skills
- Ability to liaise well with other staff at all levels
- Demonstrated flexibility in problem solving and the capability to develop and sustain highly collaborative relationships with others/ other teams.
